Submit step 3 is empty, oops

Description of a bug:
Pligg Version 6.1, site surface.ru, submit step 3 is out of order for unknown reason. You can try yourselves, login: demo, password: demo1.

Please advise:
What could be the reason and what are the possible solutions?
