Question How do you keep your content clean?

I am still working on building up my Pligg-powered site and I have been reading a lot here about spam issues.

My first line of defense is a captcha at registration.
Second line of defense is email validation.
Third line of defense is captcha on submit.

What other methods and strategies you guys employ in your effort of keeping your site clean from spam?

Do you control registrations in any way?
Do you have any other protection mechanisms (similar to captcha)?
Do you control every single submission or rely on visitors to bury articles?

    Thanks, chuckroast.

I was also looking for some tips from actual site owners like, for example whether they browse and manually check all publications, or do they have some kind of user control etc.
