Despite filters, story got high votes and on front page

I have various filters running to try to prevent spam from getting submitted and making it to the front page.

First, a story needs 3 votes to get to the front page. However, I just deleted a story that was submitted and seemed to 'instantly' get 23 votes and was on the front page. The Akismet filter also recognized it as spam. I deleted it.

How did it get such high votes (it was not a 'normal' vote I assure you)? How do I prevent it?
