Image upload MOD problem

I have problem with Yanki's Image uploader MOD and I really need your help on this.

I am working with pligg 1.01. I installed the MOD. The problem happens when I want to upload a single image to go with the story (I enabled only one single image submission for each story), the first image that I upload for the first time is working ok and the image is uploaded successfully. After that, when I want to submit a second story and choose an image to upload (a different one with a different name), I click the upload button, and when it finishes uploading, I can see the last image that I have uploaded, including it's name, and that instead of the current image that was uploaded to the server.

I have removed all caching from the server side from the mod_expires, and put a no-cache meta tag on the submit page, but it does not solve the problem. I need your help in solving this problem, I want this plugin to work very much. Can you please help me on this problem.

using: PHP 5, MySQL 5.xx on Linux OS.

Thoughts: maybe the ajax upload page is cached for some reason ?

This is probably caused by some kind of local caching with your browser, a web accelerator toolbar such as google's webaccelerator, or even possibly your ISP is caching pages so it can re-serve them out faster.

Is there anyway to check this and know what is being cached and how to solve this ?

I have checked with my hosting provider, there is no proxy server at all. Any idea why do I get the same image when I try to upload the second time. BTW if I delete and then try to upload the image again, it does work as expected. please help me on this. maybe there is someone who can identify the problem by reading the headers of the response.

What I think happend is that roboform probably field the input, even that I saw the correct filename, but behind, the input filename was the one that was saved before...

EDIT: The problem does not related to roboform.

PLEASE HELP !

I think it is nothing to do with caching, because I get the first (ever) uploaded image with the name and the image, instead of the last image

I'm sorry, but since you are the only person able to produce this problem, it's not a critical issue that we feel we should solve at this time. Best of luck figuring out the cause.

The worse thing is that if you delete photo which appearing from previous session it disappearing from my older posts they were placed. It's really annoying. It's happening when you upload photos from pc. I tested on Mozilla and Opera, the same problem.

Same problem here

Still not a clue of what can cause it, did u notice it with bigger uploads or randomly?
