Hacking attempt on Register Step 2..

i have somme problems with registration process..

i'm reciving this error "Hacking attempt on Register Step 2" when i try to register a new user, any idea?

and sometimes when i type the numbre of the image confirmation it give me a wrong answer....

help me please
